All covered Medicaid, Medicare and Home and Community-Based Waiver Services. Plus, we coordinate all of your health and long-term care services.

Care Wisconsin Support Services, including transportation, physical and occupational therapies and dental services, are covered benefits of Partnership.

Care Wisconsin Transportation Service is available on a private-pay basis for older adults or individuals with disabilities needing specialized transporation to medical or other therapeutic appointments.  Call (608) 245-3125 for more information.

Medicare-covered hospital, medical and prescription drugs. Plus, we bring you peace of mind by enhancing the quality care you are already receiving from nursing home caregivers.

Long-Term Care Services
Care Wisconsin Home Care Services and Care Wisconsin Adult Day Services provide extra support and assistance for frail older adults with medical, physical and cognitive concerns. These community-based services are available as a Partnership benefit and on a private, fee-for-service basis.
