Partnership includes covered Medicaid, Medicare and Home and Community-Based Waiver Services. Plus, we coordinate all of your health and long-term care services. Care Wisconsin Support Services, including transportation, physical and occupational therapies and dental services, are covered benefits of Partnership.

Long-Term Care Services
Care Wisconsin Home Care Services and Care Wisconsin Adult Day Services in Dane County provide extra support and assistance for frail elderly with medical, physical and cognitive concerns, and adults with disabilites. These community-based services are available as a Partnership benefit and on a private, fee-for-service basis.
