You must be eligible for Wisconsin Medicaid. If you are a Dane County resident and want to enroll in Partnership, you must: be age 18 or over and have a physical disability or be age 55 or over; have long-term care needs; meet functional and financial requirements; and be a resident of Dane County.

- be a resident of Columbia, Dodge, Jefferson or Sauk County.
