An enrollment specialist is available to meet with you and your family or talk about Partnership over the phone. Any discussion is confidential, and, of course, your decision to enroll is entirely voluntary.

Are You Eligible?
You must be eligible for Wisconsin Medicaid.

If you are a Dane County resident and want to enroll in Partnership, you must:

• be age 18 or over and have a physical disability or be age 55 or over;
• have long-term care needs;
• meet functional and financial requirements; and
• be a resident of Dane County.

If you are a Columbia, Dodge, Jefferson or Sauk County resident and want to enroll in Partnership, you must:

• be age 18 or over and have a developmental and/or physical disability or be age 65 or over;
• have long-term care needs;
• meet functional and financial requirements; and
• be a resident of Columbia, Dodge, Jefferson or Sauk County.

When Can You Enroll?
Medicaid and Medicare beneficiaries who meet eligibility requirements can enroll at any time during the year, because Partnership is a Wisconsin Family Care Partnership and Medicare Advantage Special Needs Plan program.
