You can join Partnership if you are eligible for Wisconsin Medicaid, meet certain medical or disability requirements as determined by the State and live in the plan's service area. Columbia County residents: You must be age 18 or older. Dane County residents: You must be age 55 or older. When Can You Enroll? Medicaid and Medicare beneficiaries who meet eligibility requirements can enroll at any time during the year, because Partnership is a Wisconsin Family Care Partnership and Medicare Advantage Special Needs Plan program. Services may begin January 1, 2008, or the first of the month following receipt and approval of your enrollment application. For more information, contact a Community Outreach Representative.
