• Your own Care Team — nurse practitioner, nurse, social worker and other professionals — is trained to coordinate and provide care to meet your needs.

Most importantly, your Care Team gets to know you very well and works closely to support you in your wish to live independently.

You no longer need to worry about getting to your doctor, setting up appointments and dealing with confusing medical bills. Your Care Team will coordinate all of your health and long-term care services.
