Continued independence and quality of life are common desires for ourselves and our loved ones.

For individuals with chronic conditions or functional decline, it can be difficult to meet these challenges without assistance. If you have these concerns, Partnership may be the right choice for you.

Why Partnership?

  • We partner with you, local doctors, your family and the community to create a member-centered care plan that emphasizes preventive care and supports you in achieving personal goals.
  • You can receive many health and personal care services right in your home.
  • Your own Care Team — nurse practitioner, nurse, social worker and other professionals — is trained to coordinate and provide care to meet your needs.

Most importantly, your Care Team gets to know you very well and works closely to support you in your wish to live independently.

You no longer need to worry about getting to your doctor, setting up appointments and dealing with confusing medical bills. Your Care Team will coordinate all of your health and long-term care services. You also have the opportunity to arrange, direct and purchase some or all of your long-term care services through the Self-Directed Supports option.

Partnership covers Medicaid, Medicare and Home and Community-Based Waiver Services,
such as:

  • Physician Care
  • Prescription Drugs
  • In-Home Care
  • Transportation for Medical Appointments
  • Hospital Care
  • Dental Services
  • Pre-vocational Services
  • Supported Employment

Partnership is a Wisconsin Family Care Partnership and Medicare Advantage Health Maintenance Organization (HMO) Special Needs Plan offered by Care Wisconsin Health Plan. We provide you with the health and supportive care you need to remain at home and active in the community.

What is the Cost?
Care Wisconsin receives monthly per capita (or per person) payments from Medicaid, and where applicable, from Medicare, for all eligible members.

As a member and Medicaid recipient, you pay the following for covered medical and long-term care services:

  • Monthly Premium:   $0
  • Copayments:           $0
  • Deductibles:            $0

If you have Medicaid coverage and you do not have Medicare Part D, you will pay $0 for your drugs. Individuals who are eligible to enroll in Partnership are also eligible for extra help or low-income subsidy premium assistance for Medicare Part D prescription drugs. Please see Chapter 6 of the Evidence of Coverage for information on copayments and deductibles for drugs.

Where is Partnership Available?
Care Wisconsin’s service area for Partnership includes Columbia, Dane, Dodge, Jefferson and Sauk Counties in Wisconsin.
