A Community Outreach Representative is available to meet with you and your family or to talk with you about InFocus Health over the phone. Any discussion is confidential, and, of course, your decision to enroll is entirely voluntary.

Are You Eligible?
You must be entitled to Medicare Part A and enrolled in Medicare Part B, and have resided in a qualifying nursing home for 90 days or more, or be expected to reside in a qualifying nursing home for 90 days or more, in our plan’s service area: Columbia, Dane, Dodge and Sauk Counties in Wisconsin.

Once enrolled, you must continue paying your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party.

When Can You Enroll?
Medicare beneficiaries who meet eligibility requirements can enroll at any time during the year, because InFocus Health is a Medicare Advantage Special Needs Plan. Services may begin January 1, 2008, or the first of the month following receipt and approval of your enrollment application. For more information, contact a Community Outreach Representative.
