Extra Support
As a member, you are assigned your own Care Wisconsin Nurse Practitioner (NP), a registered nurse with a master’s degree and advanced care certification, who regularly visits your nursing home to monitor your health.
Your NP is trained to detect health-related problems early, prevent minor problems from becoming serious and respond quickly to any changes in your health. Your NP also coordinates communication among your care providers, you and your family. This vital link in communication ensures everyone involved in your care understands your unique needs and respects your wishes.
